Insurance Liability for Payment: We will strive to obtain preauthorization for all services and procedures from your health insurance company when applicable. If your insurer determines that a specific service is not reasonable and necessary or is not covered under the plan, they will deny payment for that service. Under this arrangement, you are responsible for paying your co-pay, any non-covered portions, and any deductible that you have yet to cover. Additionally, if your insurance company does not pay for our services, you agree to cover the costs of the services provided in our clinic. Please be advised that timely payment is crucial to sustaining our services. In the event that payment is not received as agreed upon, we reserve the right to cancel or suspend services. It is imperative to adhere to the agreed-upon payment terms to ensure the continuity of the services provided.
